To resolve this situation, please ensure that you have configured the items in the Field Template Details correctly. Kindly refer to this guide on how to create a Catalog Item, making sure to select a Field Template to be used for the item.
Furthermore, refer to this article to add the Catalog Item to your form, paying attention to the filters, to ensure that the Catalog Item you are looking for is not filtered out.
